Care Coordinator
(Registered Nurse, Occupational Therapist, Physiotherapist or Social Worker)

Job Posting:  #029-24
Temporary, Full-Time:  70 hours bi-weekly
Effective Date:  Immediately to July 5, 2025
Advanced oral and written proficiency in English and French is essential

As a Care Coordinator, you will complete assessments to determine patient eligibility and coordinate ongoing services with service providers to address patient needs and ensure the most effective use of resources.
